The 3rd dose

You are eligible for a COVID-19 booster shot (the 3rd dose) if it has passed a certain period from the day you had completed your primary vaccination series (1st and 2nd shot) of COVID-19 vaccination. The booster doses for COVID-19 vaccination starts from those who have received their vouchers. Please apply here to issue your voucher for booster dose if you have not received your voucher yet within a certain period of time after completing the 2nd dose.

The 4th dose

You are eligible for a COVID-19 booster shot (the 4th dose) if you are 60 years and older or you are aged 18 to 59 and have underlying illness, or medical personnel , employees of senior citizen facilities, within a certain period of time after completed your booster doses (3rd shot) of COVID-19 vaccination. The issuance of voucher for the 4th dose differs depending on the municipality, so please contact your municipality.
It is possible to apply for voucher at COVID-19 Vaccine Navi, but please pay attention to that some municipalities do not accept the application for the voucher issuance at COVID-19 Vaccine Navi. Please apply here to issue your voucher.

The dose for vaccinations starting in Autumn 2022

You are eligible for a COVID-19 booster shot (vaccinations starting in Autumn 2022) if you are 5 years and older who completed the primary vaccination series and it has passed a certain period from the day you had completed your last dose. The issuance of voucher for vaccinations starting in Autumn 2022 differs depending on the municipality, so please contact your municipality.
It is possible to apply for voucher at COVID-19 Vaccine Navi, but please pay attention to that some municipalities do not accept the application for the voucher issuance at COVID-19 Vaccine Navi. Please apply here to issue your voucher.

* Your vaccination voucher is valid only for yourself. It is strictly prohibited to transfer it to anyone. You must not use it for any other purpose.

You may find a medical institution or vaccination site for your vaccination from Search Vaccination Sites. Please choose a medical institution or vaccination site and check the availability and types of vaccines.

Please check the availability before making a reservation.

Please make sure to bring and present to the reception the contents in the envelop from city and your identification (Individual Number Card, Driver's license, Health Insurance Card, etc.) when come for vaccination.
A prevaccination screening questionnaire with the vaccination voucher printed on, and a vaccination certificate are enclosed in the envelop. Please make sure to bring both for the vaccination.
* Sometimes, the vaccination voucher and the vaccination certificate are combined.

You will have the injection to your shoulder muscle. Please wear a T-shirt or other clothes that you can show your shoulders easily.
Vaccination is free of charge.
