MHLW requests those who use "Vaccination application in areas other than the registered domiciles" service in this website to register the municipalities they reside, voucher numbers, municipalities where they have their residence certificates, vaccination statuses, and reasons for application, and collects relevant information.

MHLW will collect the following information from those who apply for the booster shot (3rd dose) vaccination voucher: the city on the ceritificate of residence, name, name in furigana, date of birth, phone number, address on the ceritificate of residence, reason for application and the status of the 1st and 2nd dose of vaccination.

MHLW will collect the following information from those who apply for the booster shot (4th dose) vaccination voucher: the city on the ceritificate of residence, name, name in furigana, date of birth, phone number, address on the ceritificate of residence, reason for being eligible for the 4th dose, underlying illnesses, reason for application and the status of the 3rd dose of vaccination.

MHLW will collect the following information from those who apply for the booster shot (vaccinations starting in Autumn 2022) vaccination voucher: the city on the ceritificate of residence, name, name in furigana, date of birth, phone number, address on the ceritificate of residence, reason for application and the status of the last dose of vaccination.

MHLW uses reCAPTCHA v3 provided by Google to provide "Vaccination voucher re-issuance application", "Vaccination in areas other than the place of residence" and "Vaccination voucher for booster shot (3rd and 4th dose, and for vaccinations starting in Autumn 2022)" services. 3. Purpose of Use

MHLW uses information collected as stipulated in Section 2.(1) to facilitate the operation of services provided in this website.
(2)
MHLW shares information collected as stipulated in Section 2.(2) with municipalities and relevant extended associations for the purpose of vaccination voucher re-issuance.
(3)
MHLW shares information collected as stipulated in Section 2.(3) with municipalities and relevant extended associations for the purpose of providing vaccination in areas other than the recipients’ registered domiciles.
(4)
MHLW will provide the date colleted in Section 2.(4) for city and coalition for the sake of the booster shot (3rd dose).

MHLW will provide the date colleted in Section 2.(5) for city and coalition for the sake of the booster shot (4th dose).
(6)
MHLW will provide the date colleted in Section 2.(6) for city and coalition for the sake of the booster shot (vaccinations starting in Autumn 2022).

MHLW uses information collected as stipulated in Section 2.(6) to prevent unauthorized use by spam bots.

4. Limit of Use and Provision of Information

MHLW shall not use information collected in this site for any purposes other than those stipulated in Section 3 for itself or share such information with any third parties; except for the case where there is a disclosure request under an applicable law, a consent has been obtained from the person who provided such information, or it is permitted by applicable personal information protection laws. However, MHLW may disclose access information to this website, user attributes and other information after applying statistical processes.

5. Safety Measures

MHLW shall implement necessary measures to manage collected personal information properly, including prevention of leaks, loss, or damage of personal information. When MHLW uses a third-party contractor to handle personal information collected through this website, MHLW shall manage properly to ensure that such third-party contractor implements appropriate safety measures to protect personal information.

6. Disclosure of Staff Identity Information

MHLW will respond to requests for disclosure, correction, suspension of use of personal information collected through this website in accordance with the procedures stipulated by the applicable personal information protection law when individuals provided their personal information make such requests.
